Position Summary: Oversees the planning and coordination of admissions in the ministry. Coordinates between community referral sources and ministry availability to ensure seamless transfers based upon needs and opportunities for placement. Gathers information, including a physical assessment, to assist ministry staff in making appropriate placement decisions. Pre-qualifies for Medicare, Medicaid and third party insurance coverage.
Key Responsibilities/Essential Functions:
Receives and documents ministry inquiries from community resources and potential residents.
Gathers and documents information pertinent to resident health care and needs in preparation for admission,
Coordinates and oversees admissions to the ministry.
Assists residents with their admission and assures the coordination of services at their arrival to the facility.
Coordinates resident Medicare, Managed Care, and Private insurance Benefits including beneficiary consent, denials, explanations, and utilization.
Responsible for facility census, with the primary focus on the skilled unit.
Maintains working relationship and performs outreach activities with area resources including hospital discharge planners and other potential referral sources.
May perform other duties as assigned or requested.
JOB REQUIREMENTS:
Qualifications:
Education and/or Experience
Completion of a Practical Nurse or Registered Nurse curriculum required.
Experience in case management or acute care discharge planning preferred.
